- Statement of Accuracy and Authorization Add a statement confirming that the information provided in your Notice is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner of the exclusive right that is allegedly infringed.
- Signature Provide a physical or electronic signature of the person authorized to act on behalf of the copyright owner.

Counter-Notification
If you believe that material you posted on CN Media was removed or access to it was disabled by mistake or misidentification, you may submit a Counter-Notification. To be effective, a Counter-Notification must include the following:
- Your Contact Information Include your name, address, telephone number, and email address.
- Identification of the Removed Material Identify the material that has been removed or to which access has been disabled, and the location where the material appeared before it was removed or access was disabled.
- Statement of Good Faith Provide a statement that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.
- Consent to Jurisdiction State that you consent to the jurisdiction of the federal district court in your location (or the location of CN Media’s headquarters) and that you will accept service of process from the person who filed the original DMCA Notice or an agent of that person.
- Signature Provide your physical or electronic signature.

Procedure Following Receipt of a Valid DMCA Notice
Upon receiving a valid DMCA Notice, CN Media will:
- Remove or disable access to the allegedly infringing material.
- Notify the user or webmaster who posted the material that it has been removed or access has been disabled.
- Allow the user or webmaster to submit a Counter-Notification claiming lawful use of the removed material.
If a valid Counter-Notification is received, we may reinstate the material in question unless the copyright owner files a court action to prevent it. CN Media reserves the right to terminate access for users who are repeat infringers.
